A Message from Major Leo Phillips, the 22 Wing Family Medical Clinic Workgroup Chairperson:
The 22 Wing Family Medical Clinic is located at the rear parking lot of the CF Medical Facility at 95 Manston Crescent.
Patients can register by contacting Shirley Lirette at 705-494-2011 ext 2149. It is important to note that the Clinic will not accept patients that already have a family physician. If a patient already has a family physician but would like to become a patient of the 22 Wing Family Medical Clinic, they must inform their family physician that they no longer wish to remain as a patient and have that medical practice contact the 22 Wing Family Medical Clinic Office Coordinator to inform of the change.
The provision of medical services is currently available to all Regular Force Families, USAF Families, Class B Reservist Families, Class A Reservist and Families and Civilian members of the DND Defence Team located in the North Bay geographical region.

We are aggressively moving forward with the implementation of the Electronic Medical Records (EMR), a process that will allow for maintenance of family medical records in a manner that conforms to Federal and Provincial Privacy Laws. This capability will allow families to have controlled access to their own medical files and, more importantly, the ability to transfer them to their next posting location.

Registration forms can be completed and forwarded to the 22 Wing Family Medical Clinic Office Coordinator Attention to Shirley Lirette by one of the following: NOTE this patient registration notification is for Regular Force & USAF Families only.
- Complete form and hand deliver to the Family Medical Clinic Coordinator Reception Office between the hours of operation. The Family Medical Clinic is located at the rear parking lot of the CF Medical Facility at 95 Manston Crescent.
- Complete form and fax to Family Medical Clinic Office Coordinator at 705-494-6228.

Family Medical Clinic New Policies
The clinic has adopted some new policies regarding no show/missed appointments. Starting November 1, 2011 the clinic will ask that 24 hours notice be given when patients plan to miss an appointment. If no notice is given, the patient will recieve a warning. The next appointment the patient misses a charge will be incurred (charge will be $35.00 but can be more depending on appointment type) which needs to be paid prior to booking another appointment. Please note the clinic will take exception to unique cases and unfortunate situations. Also note the clinic will be posting a fee schedule for services that are not covered under OHIP.
